Welcome to Iowa's Immunization Registry Information System (IRIS)!

If you are an authorized user, please login using your unique combination of Organization Code, Username, and Password.
If you are a new user, please visit the training tab above and contact the Administrative User for your organization for further information. If you are from an organization requesting access for the first time, please contact the IRIS Help Desk to request enrollment information or submit the enrollment form(s) found in the Forms tab. IRIS Help Desk hours are 8:00 a.m. to 4:00 p.m. Monday through Friday, excluding state holidays. Contact the Help Desk at 800-374-3958 or email irisprogram@hhs.iowa.gov.
If you are a Standard user and need help with your IRIS account, please contact the Administrative User(s) for your organization. If you are unable to access IRIS after troubleshooting with your Admin user(s), please have the Admin user(s) contact the IRIS Help Desk at 800-374-3958 or irisprogram@hhs.iowa.gov.

Accessing Immunization Records

IRIS no longer offers a public access search component for immunization and health screening records. This functionality was removed due to continued focus on maintaining patient privacy and confidentiality of health information.

Individuals may obtain immunization records through the following options:
        1. Contact your health care provider.
        2. Complete and submit an immunization record request form to the Iowa Department of Public Health
            Immunization Program.
Individuals, parents and guardians may request immunization or health screening data by completing the IRIS Record Request Form, which is found under the Forms tab of IRIS.
