The User Information screen will be displayed after the user selects the 'Forgot Password?' button on the Portal Screen.

The user must enter their:

Org Code:          Organization code you log into IRIS with (case sensitive)

Username:         Username you log into IRIS with (case sensitive)

Email:                Email address associated with your IRIS account (case insensitive)

Select the 'Submit' button after your information is entered.

NOTE:  You will have unlimited attempts to enter the user information. If you feel you have entered the correct information and are receiving an error message, please contact the IRIS Help Desk or your organization's Administrative User.
